IN Draft_Interview and Survey Questions

This form collects information from Recovery Community Organizations (RCOs) about their services, community needs, certification experiences, grievance processes, and partnerships. The goal is to better understand the resources organizations provide, identify gaps in recovery support—particularly for youth—and gather feedback on barriers related to certification and grievance procedures. Responses will help inform improvements to support systems, training, and policy development for RCOs and the communities they serve.
