IN Follow-Up Survey Questions

This form collects follow-up information from organizations about their services, community needs, certification experiences, grievance processes, and partnerships. The goal is to better understand the resources organizations provide, identify gaps in recovery support (particularly for youth), and gather feedback on barriers related to certification and grievance procedures. Responses will help inform improvements to support systems, training, and policy development for Recovery Community Organizations (RCOs) and the communities they serve.
